Novadaq received pre-market notification 510(k) clearance from the United States Food and Drug Administration (FDA) in January 2008 for use of the SPY Imaging System during organ transplant surgery. Leveraging Novadaq’s core technology platform, SPY produces real-time images of blood flow through the vasculature indicative of reperfusion in the transplanted organ. SPY images may assist transplant surgeons in making intra-operative decisions and may potentially contribute to improved procedural and longer term outcomes. Initial independent familiarity studies performed at Baylor Medical Center demonstrated that SPY provided many potentially useful benefits in assessing intra-operative organ perfusion. Transplant surgeons performing these studies at Baylor report that based on their expertise the utility of SPY in organ transplant is clearly evident.

In liver transplant surgery, SPY images may enable the assessment of:

• patency of the vascular anastomoses (hepatic artery, portal vein, vena cava)
• organ perfusion
• bile leakage
• organ functionality via uptake of ICG and imaging of bile intra-operatively

In kidney transplant surgery, SPY images may enable the assessment of:

• patency of the vascular anastomoses
• perfusion of the renal cortex
• "washout" of ICG from renal allograft
• perfusion of the allograft ureter

In pancreas transplant surgery, SPY images may enable the assessment of:

• patency of the vascular anastomoses
• perfusion of the pancreas allograft
• perfusion of the duodenal segment of the pancreas allograft.
